Secondary Reoperation of the Non-malignant Biliary Tract Disease


Abstract
A series of 32 secondary reoperations on the biliary tracts during 7 years from June 1965 thr-ough June 1972 at Seoul National University Hospital has been analyzed, particularly with the respect to residual or recurrent stones.
The following results were obtained:
1. The reoperation rate was a 5.9% of total 538 initial biliary tract operations for the non-malignant diseases.
2. The main cause of the reoperation was a residual or recurrent stone of the bile duct in
25 cases, representing 75% of the total - and followed by the post-cholccystomy states or bile leakage in, 4 cases (12.5%), entering of ascaris into the common` bile duct in, 2 cases, and stenosis of sphincter Oddi in 1 case.
3. The cases which is thought to be preventable retrospectively were 8 cases, of which 7 cases were overlooked residual stones of the intrahepatic and common bile duct.
4. The age and sex distribution indicated a slight predominance in fifth decade and female, but 7 cases of intrahepatic stones in the third decade were remarkable.
5. The cardinal symptoms and signs were epigastric and/or RUQ pain, jaudice, fever and chills. The impairments of the liver function were noticed in a half case of the total.
6. The operative treatments such as biliary enteric anastomosis or sphincteroplasty were resulted in satisfactory outcome, except one case of death due to septicemia and one case of third reoperation.
